Who else is permitted to pick up your child from school?

To ensure the child's safety, you should always be aware if someone else is expected to pick up the child, especially if they do so on a regular basis. Asking ahead of time will save you and me as a teacher the stress of not knowing how the child will get home after school is out. 

(Attached is a link to a spreadsheet, please let me know at least two people that would possibly pick your child up from school other than yourself. Please list first, last name, and phone number)
